Individuals who have enrolled or transitioned into fee-for-service select or are auto-assigned to a Medicaid/DDD-approved Support Coordination (SC) Agency. The SC Agency then assigns a Support Coordinator. Individuals can change Support Coordination Agencies upon request. With the individual and his/her family, the Support Coordinator develops the Person Centered Planning Tool; assists the individual in identifying service needs and researching service providers; and writes the Individualized Service Plan (ISP), which authorizes services and service providers.
